I agree for my personal details (e.g. name, DOB, phone number, email etc.) to be stored in secure databases and used to send me communications about the study and to invite me to participate in future research studies.(Required) I Agree 4) I agree that the study team may access my medical, health and social care records including records held digitally and centrally by public bodies, and to analyse and store this information long-term (even in the event of my incapacity or death).
